Description

• Conduct assessments of patients in both inpatient and outpatient contexts, formulating personalized therapy plans
• Implement physical therapy techniques that emphasize mobility, strength, balance, and overall function
• Collaborate closely with nurses, physicians, case managers, and other rehabilitation professionals to optimize patient outcomes
• Guide and empower patients and their families regarding therapeutic objectives, discharge processes, and at-home exercise routines
• Maintain thorough documentation of evaluations, treatments, and patient progress as per regulatory and facility standards
• Engage in multidisciplinary team meetings and care round discussions to promote integrated care delivery

Education

• Doctorate in Physical Therapy (DPT) or parallel experience from an accredited institution

Certifications

• State licensure as a Physical Therapist
• CPR certification is preferred
• Additional specialized certifications (e.g., geriatric, neurological) are advantageous but not mandatory

Skills

• Previous experience is strongly preferred
