Job Description This person will be working directly with members to assess, plan, and coordinate care. They will have a caseload of about 75 members. Perform care management duties to assess, plan and coordinate all aspects of medical and supporting services across the continuum of care for select members to promote quality, cost effective care. Monday‑Friday 8 a.m.–5 p.m., Full time with medical benefits offered. This is an immediate Right to Hire opening. Pay range $32/hr–$35/hr salary negotiated based on relevant experience.
