Social Worker

DaVita is looking for a Social Worker to join a trust‑based, patient‑centered team. In this role, you will be a key part of each patient’s core care team, supporting and advocating for dialysis patients and helping them navigate the physical, mental, emotional, and financial demands of End‑Stage Renal Disease. Responsibilities Build meaningful and long‑term relationships with patients and their families in an outpatient setting. Collaborate with multidisciplinary team members to provide comprehensive social services. Advocate for patient rights and coordinate community resources. Assist patients and families in understanding insurance and financial assistance options. Document services and progress in patient records and maintain compliance with regulations. Requirements Meets all state required regulations to practice in the dialysis setting. Master’s degree in Social Work (MSW) with a specialization in clinical practice. Licensed LCSW or LMSW. Demonstrated knowledge of government and private insurance programs. Basic computer skills in MS Word, Excel, PowerPoint and Outlook, and functional proficiency with DaVita specific applications within 60 days.
