Complete POE and address care gaps during patient encounters. Accurately document patient data, actions, and responses to treatment. Assist with procedures and monitor patient responses. Actively engage patients, families, and caregivers in care coordination. Lead the nursing response to medical emergencies. Monitor electronic messaging related to patient or provider requests. Initiate and monitor IV therapy and medication administration. Assess health education needs and provide counseling. Conduct follow-up calls to patients for care and outreach.
