RN Care Manager

Schedule: Monday – Friday | 8:00 AM – 5:00 PM Relocation Allowance: Up to $5,000 (will be discussed further during interview) Pay Rate: Need to be Discussed During Interview Note: Client will do offer a wide variety of benefits to their employees. Key Responsibilities: Coordinates the plan of care focusing on quality, timely delivery of services, and medical necessity of a patient’s admission, including what level of care is appropriate. Assesses, plans, implements, monitors, and evaluates options and services to meet individual’s health care needs and the family systems that support the patient. Care Manager seeks positive health outcomes by ensuring well‑orchestrated, coordinated care focused on quality, efficiency and safety. This position maintains an open, approachable manner treating others fairly and respectfully preserving self-confidence and dignity as well as show regard for their opinion(s).
