Behavioral Health Care Team Manager

About this position Care Team Manager – Behavioral Health Location: Detroit, MI metro area FLSA Status: Exempt Job Status: Full Time Work Model: Hybrid Work Schedule: Monday–Friday with occasional evenings and/or weekends Vehicle Required: Yes Amount of Travel Required: Occasional domestic travel in Indiana and to Minneapolis, MN Positions Supervised: Behavioral Health Community Guides Reports To: Assistant Director of Care Team Operations Reema Health is a social care and behavioral health provider that partners clinical and non‑clinical staff to offer community‑based support for individuals with serious mental illness and substance use disorder. The Care Team Manager leads the Care Team Program, partners with clinical psychotherapists and prescribers, and empowers Community Guides to deliver high‑quality, holistic support to members. Responsibilities Lead the team by establishing a deep understanding of Reema’s program goals, metrics, and expectations for Community Guides. Champion change management, leading hybrid Care Teams through ambiguity and continuous innovation. Develop and sustain structures and processes that promote communication and accountability, including 1:1 meetings, goal setting, team meetings, multidisciplinary case reviews, community volunteering, and team‑building events. Provide regular feedback and recognition on Community Guide performance. Partner with clinicians to align services, manage multidisciplinary workflows, and ensure high‑quality delivery. Facilitate weekly check‑ins on staffing and metrics, report updates, and set monthly Care Team goals with the Director of Clinical Care Team Operations. Supervise and support Guides, research member resources, and champion outreach efforts. Participate in hiring, onboarding, and ongoing training. Maintain PHI confidentiality and uphold HIPAA and software‑security policies. Build and nurture relationships with local community partners along the mental‑health continuum. Spend 5–10 % of time providing direct member support through cold outreach, messaging, phone, home visits, and referral coordination. Adapt responsibilities as business needs evolve. Education, Experience and Skills Bachelor’s degree in Social Work, Human Services, Psychology, Health Sciences, or related field (or equivalent experience). Three years of people‑management experience in healthcare, mental‑health, community‑based, or social‑work settings. Five years’ direct experience with the local population served and comprehensive community knowledge. Strong critical‑thinking, strategic‑thinking, time‑management, and prioritization skills. Knowledge of crisis planning, case management, and behavioral‑health concepts. Experience working with diverse ethnic backgrounds, serious chronic mental illness, chemical‑health needs, adolescents, and hard‑to‑reach populations such as homelessness. Proven ability to navigate ambiguity, solve problems creatively, and build relationships quickly. Experience collaborating in multidisciplinary teams; familiarity with video‑conferencing and Microsoft Office suite (Google Workspace preferred). Qualifications & Position Requirements 18 years of age or older. Valid driver’s license, vehicle insurance, and satisfactory driving record. Ability to pass a pre‑employment drug screening and background check. Compliance with federal, state, and local patient‑confidentiality regulations (HIPAA). Comfort with daily use of technology‑based tools. Occasional lifting and carrying up to 25 pounds.
