HCBS Coordinator – Home and Community-Based Services

Salary: $75,000 – $85,000 Competitive, Based on Experience
Job Type: Full-Time | In-Person / Office-Based with Community Visits
Location: Brooklyn, NY

About the Role

Doral Health & Wellness is seeking a detail-oriented and compassionate HCBS Coordinator to support clients enrolled in Home and Community-Based Services (HCBS). This role focuses on care coordination, Medicaid compliance, documentation, and communication with clients, families, and service providers to ensure high-quality community-based care.

Key Responsibilities

Coordinate HCBS services and supports for enrolled clients
Monitor client progress and ensure individualized service plans are followed
Communicate with clients, families, and service providers
Maintain accurate documentation in EHR/case management systems
Assist with scheduling appointments and community services
Support compliance with Medicaid and HCBS regulations

Qualifications

High school diploma required; Bachelor’s degree preferred
Experience in HCBS, Medicaid, case management, or community-based services preferred
Strong organizational, communication, and time-management skills
Familiarity with EHR or care coordination systems
