Pay Range: $40.07 - $48.10 per hour, depending on prior experience.

• Graduation from an accredited nursing program; a Bachelor's degree is preferred.
• Current RN license in the state of Colorado.
• A minimum of two years of nursing experience, including long-term care, dementia care, or acute care; home care experience is preferred.
• CPR certification.

As a Registered Nurse (RN) , you will:

• Manage cases, coordinate services, supervise, and implement professional and supportive care for clients, utilizing nursing theories and processes in line with the Nurse Practice Act.
• Review client referrals and respond to inquiries as needed.
• Conduct client assessments according to agency standards to ensure effective home care services.
• Independently direct, plan, and initiate appropriate actions in home care situations.
• Perform duties in accordance with agency policies, procedures, and professional standards.
• Complete additional tasks as assigned by the Director of Clinical Services.

Our organization is a Medicare/Medicaid-certified home health care agency located in Montrose, CO. We have a dedicated team of nurses, rehabilitation therapists, and home health aides focused on providing personalized healthcare with compassion and empathy. Our expertise lies in caring for older adults, delivering in-home health services across Delta, Montrose, and Ouray counties.

• Required Experience: 2 years in nursing, including long-term care, dementia care, or acute care.
• Preferred Experience: Home care experience.
• Preferred Education: Bachelor's degree in Nursing or a related field.

• Required: CPR/First Aid certification and Registered Nurse license.
