PermiaCare is seeking a Jail Continuity of Care Case Manager in Odessa, TX to connect incarcerated individuals with mental health and substance use services. This role involves coordinating referrals for inmates and maintaining accurate tracking of the data related to services provided. Ideal candidates will have a Bachelor's degree in a relevant field and must possess knowledge of mental illness treatment and Medicaid standards.
