California Licensed Clinical Pharmacist

Delivering pharmaceutical care services at the patient and population level, the full-time California Licensed Clinical Pharmacist will analyze data to improve medication use, provide clinical services, and educate patients and providers while working remotely. Key responsibilities Provide clinical services, medication literature evaluation, and drug information services in accordance with regulations Analyze large data sets to identify strategies that enhance medication use across the care continuum Implement patient care plans based on health status and risk factors, collaborating with healthcare teams Required qualifications PharmD from an accredited pharmacy school Registered Pharmacist (RPH) license Minimum of 5 years of recent relevant experience Completion of a post-graduate year one (PGY1) residency or 3 years of experience in a managed care or health-system environment Knowledge of legal requirements and accreditation standards related to pharmacy practice
