RN Care Transition Specialist — Hospital Discharge

IntelliResume

Molina Healthcare is seeking a Care Review Clinician (RN) to support Medicaid members transitioning from inpatient to discharge. You will perform telephonic outreach and in-person visits while members are hospitalized, ensuring smooth care transitions.

The role requires an active RN license with 2+ years in acute hospital or managed care settings. You will work with a multidisciplinary team in the Mid Florida, FL area, following UM policies and delivering quality patient care.
