Pure Home Health Care is looking to bring Physical Therapists home

Do you have a passion for helping patients develop, recover, improve, and maintain the skills needed for daily living? Pure Home Health Care (PHHC) is seeking to hire a Physical Therapist to deliver care to patients in their place of residence. You will help people fully engage in their lives by providing exercises, strategies, adaptations, adjustments, and tools that help them function more independently.

No two days are the same. Every patient has different needs and challenges, and every residence provides a change of scenery. You’ll be following the plan of care and providing services that help reach goals set forth by their physician.

Duties:

· Provide a variety of physical therapy services to clients in their residence in accordance with physician's authorized plan of care

· Determine each patient’s functional status and functional limitations in regards to activities of daily living.

· When a physical therapy or certified home health aide is assigned, prepare written exercise program; supervises, instructs and directs in accordance with regulatory guidelines, Agency and accreditation standards

· Determine patient’s equipment needs

· Evaluate and regularly re-evaluate the physical therapy needs of the patient, and assess their continual care needs

· Initiate diagnostic, preventive, and rehabilitative procedures as appropriate, and make referrals to other disciplines as indicated by the needs of the patient.

· Observe signs and symptoms and report changes in the patient's condition to the appropriate health professionals as needed

· Develop, prepare, and maintain individualized patient care progress records and submit documentation within 48 hours of visit

· Keep current in physical therapy trends and knowledge and participate in in-service programs

Qualifications:

1. License to practice as a Physical Therapist in the State of Michigan

2. Has graduated from a physical therapy curriculum approved by:

A. The American Physical Therapy Association, or

B. The Committee on Allied Health Education and Accreditation of the American Medical Association, or

C. The Council on Medical Education of the American Medical Association and the American Physical Therapy Association; or

3. Graduated prior to January 1, 1966:

A. Was admitted to membership by the American Physical Therapy Association, or

B. Was admitted to registration by the American Registry of Physical Therapist, or

C. Has graduated from a physical therapy curriculum in a 4-year college or university approved by a State department of education; or

4. Has two years of appropriate experience as a physical therapist, and has achieved a satisfactory grade on a proficiency examination conducted, approved, or sponsored by the U.S. Public Health Service except that such determinations of proficiency do not apply with respect to persons initially licensed by a State or seeking qualification as a physical therapist after December 31,1977; or

5. Was licensed or registered prior to January 1, 1966, and prior to January 1, 1970, had 15 years of full-time experience in the treatment of illness or injury through the practice of physical therapy in which services were rendered under the order and direction of attending and referring Doctor of Medicine or osteopathy; or

6. If trained outside of the United States:

A. Graduated since 1928 from a physical therapy curriculum approved in the country in which the curriculum was located and in which there is a member organization of the World Confederation for Physical Therapy.

B. Meets the requirements for membership in a member organization of the World Conference for Physical Therapy.

7. At least one year of experience in a health care or rehabilitative facility with demonstrated competency in the practice of occupational therapy.

8. Current Michigan Operator's License.

9. Has reliable transportation and willing to drive in all kinds of weather.

10. Willing to work the hours required by the Agency, including evening and weekend hours as agreed upon, and to maintain a flexible schedule.

11. Health and stamina sufficient to perform the job.
