Duties and Responsibilities
• Performs ongoing, systematic assessment and data collection.
• Focuses on the physiological, psychological, and cognitive status of patients.
• Analyzes assessment data to determine appropriate nursing diagnoses.
• Identifies expected outcomes that are individualized to each patient.
• Develops a goal-directed, prioritized plan of care based on nursing diagnoses and desired outcomes.
• Identifies patient and/or significant other learning needs and implements appropriate interventions.
• Delivers care in a knowledgeable, skillful, consistent, and continuous manner.
• Completes documentation in a timely, accurate, and concise manner in both unit systems and patient records.
• Evaluates the effectiveness of care provided, including:
o Personal performance
o Contributions of healthcare team members
o Impact of systems, environment, and equipment
• Assesses patient progress toward established outcomes.

Professional Performance
• Demonstrates awareness and application of patient safety standards.
• Responds effectively in emergency situations by following established protocols, remaining calm, notifying appropriate personnel, and documenting events.
• Applies a strong clinical knowledge base in care delivery and decision-making.
• Adheres to all Medical Center, Division of Nursing, and unit-specific policies and procedures.
• Consistently incorporates hourly rounding into patient care routines.
