As the Physical Therapist, you will use discretion and independent judgment to provide age‑appropriate services for hospice, including comprehensive assessment, direct treatment, development of a home exercise program, patient/family instruction, participation in Interdisciplinary Group, evaluation and selection of adaptive equipment, and documentation of services according to hospice standards. Primary Responsibilities: Initiate physical therapy evaluation according to physician orders and Plan of Care for the purpose of symptom control or to enable the patient to maintain activities of daily living and basic functional skills. Document in the patient's clinical record each visit and give a report including the patient's reaction to treatment and any changes in the patient's status, comfort level and follow‑up to physician and Administrator using the Care Management process. Participate in the Interdisciplinary Group and provide applicable input for the patient's Plan of Care as needed. Instruct and inform interdisciplinary group members, the patient, the physician and the family about functional limitations and progress toward goals as defined by the Plan of Care daily or no less than two weeks' time. Prepare a written discharge summary when physical therapy services are discontinued with a copy retained in the patient's clinical record and a copy forwarded to the patient's physician upon discharge from the physical therapy service. Instruct, when appropriate, patient, family and other hospice team members in certain phases of physical therapy in which they may work with the patient. Ability to function in any home situation regardless of age, race, creed, color, sex, disability or financial condition of the client. Required Qualifications: Current Physical Therapy licensure in the state of Oregon. Current CPR certification or ability to complete within 90 days of hire. Current driver's license and vehicle insurance, access to a dependable vehicle or public transportation. Pay is based on several factors including but not limited to local labor markets, education, work experience, certifications, etc. In addition to your salary, we offer benefits such as a comprehensive benefits package, incentive and recognition programs, equity stock purchase and 401k contribution (subject to eligibility requirements). The salary for this role will range from $80,864 to $121,296 annually based on full‑time employment. We comply with all minimum wage laws as applicable.
