Behavioral Health Case Manager - Community Access Advocate
Zufall Health
Zufall Health is seeking a Case Manager to support patients and families in navigating clinical and social services. You will work with the Care Team to identify needs, address barriers, and coordinate referrals to behavioral health and enabling services. Local travel and occasional after-hours attendance may be required. Responsibilities include following up on missed appointments, documenting progress, and assisting with transportation, translation, and charity care applications to improve #J-18808-Ljbffr Zufall Health
