Key Responsibilities

• Evaluate and treat patients in accordance with individualized physical therapy plans of care
• Deliver skilled physical therapy services during day-shift hours, Monday through Friday
• Document patient progress and maintain accurate clinical records in compliance with facility standards
• Wear a valid photo badge and present a state or federal issued photo identification at all times while on site

Required Qualifications

• Active Physical Therapist (PT) license in the state of Washington
• Minimum 35-hour weekly availability to meet guaranteed hours requirement
• Ability to provide a valid state or federal issued photo identification upon arrival at the facility
