Job Description:

Seeking a skilled Occupational Therapist for a Home Health position providing coverage across north-eastern Sacramento into Folsom and Woodland - Yolo county service area. The role involves conducting an average of four visits per day, including evaluations and revisits. A current driver's license and proof of automobile insurance are required.

Responsibilities:

• Conduct evaluations and revisits for patients in their homes
• Travel within the designated coverage area to provide care

Requirements:

• 2 years of experience in a healthcare setting
• Required certifications: BLS and NPI number
• California OT license
• Current driver's license and proof of automobile insurance
• Charting experience with Cerner system is required
• Submission requires 2 supervisory references, a copy of the driver's license, and name verification
