FlexStaff is seeking a Registered Nurse (RN) to work full-time as a Nurse Case Manager for an Assisted Living facility in Queens, NY. You will lead clinical assessments, coordinate care plans, and liaise with residents, families, providers, and staff to promote safety, well‑being, and independence. You will coordinate services, manage transitions of care, and ensure compliance with NYSDOH regulations while documenting in the EMR. #J-18808-Ljbffr Northwell Health
