Registered Nurse - Acute Care Transition Unit

In this role, you’ll deliver high-quality, patient-centered care to adults with complex and evolving needs, focusing on safe transitions between levels of care. Responsibilities include comprehensive assessments, IV and medication management, monitoring changes in condition, coordinating discharges, and educating patients and families.
