Physical Therapist Position

We are seeking a physical therapist to support acute care patient needs at Fairview Southdale Hospital in Edina!

The physical therapist will work with a variety of patient populations, including, but not limited to, ortho, neuro, general medical, general surgical, cardiac, oncology and medically complex. In collaboration with patients and families, the physical therapist is responsible for providing safe and effective delivery of patient care within scope of practice. This includes examination, evaluation, diagnosis, planning, intervention and establishing outcomes.

• FTE: 1.0 (80 hrs per pay period) Baylor Position.
• Schedule: Monday, Tuesday and Friday: 8:00am - 5:30pm and two full weekends (Saturday and Sunday) per month for 9-hour shifts (8:00am - 5:30pm) and one holiday per year.

Fairview Rehabilitation offers a broad range of services that serve patients across 10 acute-care hospitals, 3 post-acute settings and 70 outpatient adult and pediatric therapy clinics. Consisting of Physical, Occupational and Speech Therapy as well as Audiology and Cardiac & Pulmonary Rehab, our therapists collaborate with colleagues in all medical settings and offer dozens of specialty programs.

Responsibilities

• The physical therapist partners with the patient and family while performing the Evaluation/Re-evaluation
• Gathers pertinent data
• Performs examination
• Determines treatment diagnosis
• Develops an individualized plan of care through collaboration with the patient and family
• Analyzes data to identify needs and potential risks
• Determines prognosis for recovery
• Establishes individualized goals with patient and/or caregiver
• Individualizes plan of care considering developmental level and cultural differences
• Implements and modifies plan of care based on reassessment and patient response
• Coordinates care and communicates effectively with interdisciplinary team for all aspects of patient care
• Establishes appropriate discharge plan
• Partners with patients and families to provide skilled intervention
• Implements plan of care
• Alters treatment/plan of care to reflect change in patient status and response to treatment
• Assesses patient and/or caregiver learning needs and provides appropriate education

Required Qualifications

• Physical Therapy License in the state of Minnesota or temporary license in the state of Minnesota
• Basic Life Support (American Heart Association or Red Cross)

Preferred Qualifications

• Doctorate of Physical Therapy
• 1+ year experience
• 1+ year acute care experience

Compensation Disclaimer

The posted pay range is for a 40-hour workweek (1.0 FTE). The actual rate of pay offered within this range may depend on several factors, such as FTE, skills, knowledge, relevant education, experience, and market conditions. Additionally, our organization values pay equity and considers the internal equity of our team when making any offer. Hiring at the maximum of the range is not typical.
