About the Position

Job Title: RN Home Health Profession: RN Home Health Specialty: RN Home Health Duration: 13 weeks Shift: 08:00 AM - 04:30 PM Hours per Shift: 8 Experience: Minimum of 1 year home care experience, 1 year case management experience License: Valid RN license Certifications: None specified Must-Have: Ability to travel to cases as assigned Description: All applicants must have 1 year of certified home care experience. 1 year of case management experience is required. Ability to travel to assigned cases is necessary. Weekend rotations occur once every 6 weeks. As a Home Health RN, responsibilities include assessing patient conditions. Implementing care plans is also a key duty. Providing ongoing education and support to patients and their families is essential. Patient care must align with organizational policy and community health practice standards. Admission processes and case management duties are required in accordance with policies and standards of practice. Thorough and timely electronic documentation and interdisciplinary communication are expected.
