Self Harmony Counseling is seeking a dedicated, empathetic Licensed Master Social Worker (LMSW) to join our team in a full-time capacity, focused on providing essential social work services to seniors and their families in our community. In this position, you will play a critical role in enhancing the quality of life for our elderly clients by offering comprehensive support, advocacy, and coordination of care.

This is a remote position, allowing flexibility while maintaining strong ties with our clients at local schools and community settings. You’ll be part of a collaborative team dedicated to serving some of the most vulnerable members of our society. The work hours are Monday to Friday, from 8:00 AM to 4:30 PM, with occasional evening or weekend hours as needed to accommodate client availability and emergencies. You may also be required to be on call to support urgent situations outside of regular hours.

• Conduct in-depth assessments of each senior client's psychosocial needs, weighing emotional, mental, and social factors that impact their well-being.

• Develop and implement personalized care plans in collaboration with clients and their families, ensuring alignment with their specific goals and preferences.

• Provide individual and group counseling sessions, offering emotional support as clients navigate life transitions such as health changes, loss, or relocation.

• Act as an advocate for clients' rights, navigating complex systems to ensure their access to necessary resources, including healthcare and community services.

• Work alongside interdisciplinary teams that include healthcare providers, caregivers, and educators to enhance client care and service delivery.

• Regularly visit clients to monitor their living conditions, reassessing their needs and proactively addressing potential safety concerns.
\n
• Facilitate effective communication among family members involved in the care of the elderly client, resolving conflicts and misunderstandings as necessary.
\n
• Provide educational resources to clients and their families about available community programs, support groups, and engagement activities.
\n
• Maintain accurate and detailed documentation of client interactions, care plans, and progress notes in accordance with ethical guidelines.

• Stay informed on current social work practices and policies, ensuring full compliance with state regulations and ethical standards.

The primary focus of this role is to support seniors, typically aged 65 and older, who may be facing complex emotional, psychological, and social challenges. The work involves engaging directly with clients within their living environments, schools where they may partake in group activities, and community facilities. You will also work with families to educate them about the aging process and available resources to enhance their support systems.

• Master's degree in Social Work from an accredited institution is required.

• Current state licensure as a Licensed Master Social Worker (LMSW).

• A minimum of two years of professional experience in social work, ideally within a community setting or focused on geriatric care.

• In-depth understanding of social work principles, particularly those related to aging issues, along with knowledge of local resources.

• Demonstrated ability to conduct assessments, provide counseling, and implement care plans tailored to individuals' needs.

• Excellent communication and interpersonal skills to foster relationships with clients, families, and team members.
\n
• Proficiency in using electronic health records (EHR) and relevant technology to document client interactions effectively.

• Experience working with interdisciplinary teams in a healthcare or community-based setting.

• Familiarity with community resources and support services specific to seniors.

• Previous roles that involved advocacy, crisis intervention, and conflict resolution among families.

The compensation for this full-time position ranges from $60,000 to $100,000 per year, depending on experience and qualifications.
