Registered Nurse (RN) – Home Health

Schedule: Monday–Friday, day shift
Hours: 40 hours guaranteed
On-Call: None
License: WV RN or Compact
Productivity: Minimum 30 points/week
Mileage: $0.49/mile
Counties: Wood, Wirt & Calhoun

Job Overview

Seeking an experienced Home Health RN to assess high-risk patients, develop individualized care plans, and coordinate services to support positive outcomes. The RN will collaborate with physicians, social workers, and other healthcare professionals to ensure patients receive appropriate, cost-effective care.

Key Responsibilities

Perform comprehensive patient assessments and develop care plans.
Coordinate care with providers, social workers, discharge planners, and community resources.
Manage care transitions and connect patients with medical and non-medical support.
Monitor clinical, functional, and care outcomes and adjust plans as needed.
Identify opportunities for health promotion and disease prevention.
Advocate for patients while maintaining privacy and confidentiality.
Document patient care and outcomes accurately.

Requirements

Active WV or Compact RN License
Associate or Bachelor's degree in Nursing or related field
Home Health experience required
OASIS and HCHB experience required
Strong clinical assessment and critical-thinking skills
Case Management certification
Ability to meet a minimum 30-point weekly productivity requirement
