Job Title: Case Manager Profession: Case Manager Specialty: Case Management Duration: 13 weeks Shift: Monday to Friday Hours per Shift: 8:00 AM - 4:30 PM Experience: Minimum of 1 year in a hospital setting License: Must be licensed in the state Certifications: None specified Must-Have: Experience with EMR systems (Cerner preferred) Description: The Case Manager will work in an acute rehab unit. This role involves managing a caseload of approximately 14 patients. The schedule for this position is Monday through Friday from 8:00 AM to 4:30 PM. Candidates must have at least one year of case management experience in a hospital setting. Business casual attire is required for this position. This position does not require floating to other units. All qualified candidates are encouraged to apply.
