Evening Shift LPN (Licensed Practical Nurse)

Job Summary:

Mystic Meadows Rehab & Nursing Center, a premier provider of comprehensive healthcare services, is seeking a dedicated and compassionate LPN (Licensed Practical Nurse) to join our dynamic team. Responsibilities:

Provide high-quality nursing care to our residents, including administering medications, wound care, and vital signs monitoring
Assist with daily living activities, such as bathing, dressing, and feeding, as needed
Develop and implement individualized care plans in collaboration with the interdisciplinary care team
Communicate effectively with residents, families, and healthcare professionals to ensure seamless coordination of care
Participate in quality improvement initiatives to enhance the overall resident experience
Demonstrate a strong understanding of resident autonomy and dignity, promoting a culture of respect and compassion

About the Role:

As an Evening Shift LPN, you will work a flexible schedule, including evenings, weekends, and holidays. You will have the opportunity to work closely with our experienced team of healthcare professionals, including registered nurses, therapists, and physicians.
