First Choice Home Health is seeking a compassionate, skilled Physical Therapist to join our growing home health team in the Phoenix and East Valley area. Service Area: Phoenix, Chandler, Gilbert, Mesa, Tempe, and surrounding East Valley communities.

What You'll Do:

• Evaluate patients in the home setting and develop individualized treatment plans
• Provide skilled physical therapy interventions to improve mobility, strength, balance, safety, and function
• Educate patients, families, and caregivers on exercises, mobility strategies, and fall prevention
• Monitor progress toward goals and modify treatment plans as needed
• Collaborate with physicians, nurses, therapists, and office staff to coordinate care
• Complete accurate and timely documentation
• Support patients in achieving the highest possible level of independence at home

Pay & Benefits:

• $80$125 per visit, depending on visit type
• Mileage reimbursement

Qualifications:

• Active Arizona Physical Therapist license
• Valid driver's license
• Reliable transportation
• Ability to travel throughout assigned territory
• Strong clinical, communication, and documentation skills

Preferred:

• Home health experience
• Residence in Phoenix or the East Valley, including Chandler, Gilbert, Mesa, or nearby communities
