The applicant must have a bachelor's degree in a Human Services field, including psychology, social work, nursing, counseling, sociology or a related field. Assists older adults and persons with disabilities with accessing a variety of public and government benefits and programs; ensures the needs of home-bound consumers are met. Serves as case manager for identified consumers. The hiring salary for this position is $55,910 annually; full salary range for this position is $55,910 - $96,179 annually.

Essential Functions

• May determine consumer eligibility to participate in home-based case management services;
• Assists older adults and persons with disabilities in obtaining access to and qualifying for social, financial, and community benefits;
• Provides guidance to seniors and families through MAP Senior Information Assistance and State Health Insurance Assistance programs;
• Makes referrals, organizes and schedules special assistance and benefits programs for older adults;
• Maintains appropriate statistical data and case records;
• Establishes and maintains good relationships with other relevant community service agencies;
• May visit homes, hospitals, etc., as necessary;
• Conducts outreach and public education events;
• Performs other duties as assigned.

Required Knowledge, Skills, and Abilities

• Ability to gain thorough knowledge of St. Mary's County Government policies and procedures;
• Ability to effectively communicate with staff, consumers, their families, other community agencies, and the public in a courteous manner;
• Thorough knowledge of government and community programs, services, and resources available to senior citizens;
• Ability to work independently on tasks and maintain records;
• Ability to plan, organize, and execute essential tasks with minimum instruction or review.

Education and Experience

Physical and Environmental Conditions:

• Work requires no unusual demand for physical effort.
• Work environment involves moderate risks or discomforts which require safety precautions when visiting consumers' homes.

• Must successfully pass background investigation with favorable results;
• Must pass a pre-employment drug test.
