Do you currently have professional relationships with hospital case managers and/or discharge planners that have generated skilled nursing facility (SNF) referrals?We are seeking a Hospital Marketing Liaison (SNF) to support hospital referrals and patient admissions for a skilled nursing and rehabilitation facility in Tampa, FL. Key Responsibilities
Support hospital referrals and level-of-care placement decisions
Build and maintain relationships with hospital case managers, discharge planners, physicians, and care teams
Respond to hospital inquiries and support timely admissions
Maintain regular communication and on-site presence with hospital partners
Help improve coordination of patient transitions into the facility
4+ years in clinical liaison, hospital admissions, or healthcare business development
Strong understanding of post-acute care and SNF placement criteria
Experience working with hospital case managers and discharge planners preferred
Strong communication, clinical judgment, and follow-through skills
Competitive pay: $80,000 - $110,000 (DOE)
