Clinical Pharmacist

Role Overview The Pharmacist implements pharmaceutical services within a Medical Center, dispensing medications accurately and performing clinical functions such as therapeutic drug monitoring and patient-specific dosing. Key Responsibilities Check all medications leaving the department to ensure quality assurance. Dispense medications based on physician orders, verifying dosages for patient appropriateness. Communicate with healthcare professionals regarding medication orders, drug interactions, allergies, and dosing questions. Utilize computer systems and reference materials to support medication selection and therapy management. Oversee medication preparation, including IV admixtures, unit doses, and refills, ensuring accuracy in filling and labeling. Participate in departmental projects, policy updates, and training programs to maintain high standards of pharmacy practice. Monitor patients for adverse drug reactions and ensure safe medication use. Work with pharmacy technicians and oversee systems like Omnicell and HBOC for medication management. Dosed medications using pharmacokinetic services and managed investigational drugs and narcotic control as needed. Qualifications & Skills Licensed Pharmacist with a Bachelor's Degree in Pharmacy. Knowledge of chemotherapy agents, hyperalimentation, IV admixture, narcotic control, investigational drugs, and therapeutic drug monitoring. Excellent communication skills for collaboration with healthcare professionals. Competence in working with pharmacy computer systems, documenting in patient charts, and clinical dosing. #J-18808-Ljbffr
