Job Type: Full-Time / Part-Time
Shift: Days (Mon-Fri) occasional weekend visits based on patient needs
Setting: Home Health (Patient Homes)

Job Summary:


We are seeking a compassionate Occupational Therapist to provide skilled home health therapy services to patients across Northern Virginia. The therapist will help patients improve independence in daily activities, enhance safety, and support recovery in their home environment.

Roles and Responsibilities

• Conduct initial, ongoing, and discharge occupational therapy evaluations in patients' homes
• Develop and implement individualized therapy care plans
• Provide therapy focused on activities of daily living (ADLs) and instrumental activities of daily living (IADLs)
• Train patients on adaptive techniques and the use of assistive devices
• Provide upper extremity rehabilitation and functional mobility training
• Educate patients and caregivers on fall prevention, safety, and home modifications
• Monitor patient progress and adjust treatment plans as needed
• Collaborate with physicians, nurses, and interdisciplinary healthcare teams
• Accurately document patient visits in the Electronic Health Record (EHR) system
• Participate in care planning meetings and interdisciplinary case conferences
• Ensure compliance with Medicare, Medicaid, and regulatory standards

Qualification, Licensure, Experience

• New Graduates Accepted
• Active Virginia Occupational Therapist License
• CPR Certification and TB Screening required
• Minimum 1 year of clinical experience preferred (home health or outpatient therapy preferred)
• Reliable transportation for home health visits
• Familiarity with Electronic Health Records (EHR)
• Strong patient assessment and care planning skills
• OASIS documentation experience strongly preferred
