Key Responsibilities Conduct comprehensive clinical assessments to evaluate patients' mental health needs, incorporating psychosocial histories and emotional wellbeing. Develop and implement individualized treatment plans that reflect each patient's goals and needs. Utilize evidence-based therapeutic interventions, such as cognitive-behavioral therapy (CBT) and mindfulness strategies, tailored for patients coping with terminal illnesses. Maintain meticulous clinical documentation that complies with regulatory standards while ensuring confidentiality and ethical considerations. Collaborate closely with a multidisciplinary team, including physicians, nurses, and other healthcare providers, to coordinate patient care and address comprehensive health objectives. Provide grief counseling and support to families, helping them understand and cope with the emotional aspects of end-of-life issues. Regularly review and update treatment plans based on patient progress and emerging challenges. Patient Population / Client Focus Your work will primarily focus on patients referred to hospice care, facing advanced illnesses or terminal conditions. This population often requires not only clinical interventions but also emotional and psychological support that respects their values and wishes. You will engage with a diverse group of patients and families, fostering a trusting and empathic relationship that is essential for effective counseling. Qualifications A Bachelor's degree in Social Work from an accredited institution. A valid Licensed Clinical Social Worker (LCSW) license in the state of Arizona. Demonstration of knowledge and experience in solution-focused psychotherapy interventions. Strong communication and interpersonal skills, enabling effective interactions with patients, families, and team members. Preferred Experience Previous experience in hospice or palliative care settings is preferred, providing insights into the unique challenges faced by terminally ill patients and their families. Familiarity with electronic health records (EHR) and documentation requirements enhances workflow efficiency. Experience in crisis intervention and support is highly valued. Compensation and Benefits The annual salary for this position ranges from USD 80,000 to 100,000, commensurate with experience and qualifications.
