Location Chicago, IL Salary Range $80,000–$90,000 annually. Job Description As a clinically competent, registered professional nurse, you will coordinate multidisciplinary care to patients using the nursing process. Responsibilities Effectively collaborate with the patient, significant others, and other members of the healthcare team while maintaining ANA standards of professional nursing practice. Assess and reassess the patient and family for ongoing care needs with knowledge of high‑risk criteria and initiate referrals to appropriate resources in a timely manner. For all new patients, collect and analyze data, identify and coordinate appropriate multidisciplinary care and appointments; triage this information and schedule patients accordingly. Collaborate with patient, family, physicians, and ancillary staff and coordinate new patients for multidisciplinary clinics. Assist in creating a plan of care that indicates collaboration and communication. Research – utilize the best available evidence, including research findings to guide practice decisions. Aware of available, current research trials and collaborate with research staff for qualifying patients. Serve as a resource to non‑clinical staff regarding prospective patients calling for information. State rationale for clinical decisions based on critical thinking. Perform all aspects of patient care in a manner that optimizes patient safety and reduces likelihood of medical/health‑care errors. Identify learning needs, abilities, preferences and readiness; assess support systems, coping mechanisms and barriers to learning, making referrals as appropriate. Educate patient, family and caregivers regarding diagnosis, process, therapies and testing. Assist with transitions in care and providers. Demonstrate an interest in personal development and growth beyond routine expected educational sessions. Required Current experience within the past five years. Completion of an accredited Registered Nurse program with a BSN and RN license in Illinois (LIC RN). CERT BLS. Basic computer knowledge. Ability to communicate effectively in English, both verbally and in writing. Excellent customer service skills. Consistently works within the framework of the Illinois Practice Act for Nursing and ANA Standards of Professional Nursing Practice. Ability to assess and determine situations requiring an interpreter and use interpreter services effectively. Preferred One year of experience in acute care in a hospital setting or one year in an outpatient setting. Desired to be within the last four years of employment. Prior case management or patient navigation experience in an acute or outpatient setting. Basic EPIC EMR knowledge.
