Schedule & Coverage

This is a full-time, driving position serving Wooster and surrounding communities .

Schedule includes:

• Three weekdays: 4:00 PM – 12:00 AM
• Every other weekend: 8:00 AM – 8:00 PM
• Participation in a rotating holiday schedule is required.

What You'll Do

As an LPN on our Individualized Care team, you'll provide skilled nursing care under the direction of a Registered Nurse while helping patients remain comfortable in their homes or care facilities.

Responsibilities include:

• Deliver compassionate, patient-centered nursing care in home and facility settings.
• Monitor patient conditions and report changes to the supervising RN.
• Administer treatments and medications as directed.
• Accurately document care using electronic medical records.
• Educate and support patients and families throughout their hospice journey.
• Collaborate with an interdisciplinary care team to ensure each patient receives individualized care.

What We're Looking For

Qualified candidates will have:

• Current Ohio Licensed Practical Nurse (LPN) license in good standing.
• At least one year of nursing experience preferred.
• Current CPR certification.
• Basic computer skills for electronic documentation.
• A valid driver's license, reliable transportation, and state-required automobile insurance.
• Ability to travel throughout the service area in various weather conditions.
• Successful completion of a criminal background check and 10-panel drug screening.
