Key responsibilities include: Maintenance and review of patient medication profiles. Reviewing and verifying physician medication orders and patient medical record. Checking of IV products. Interacting with healthcare professionals and providers to promote safe medication use and prevent inappropriate or misuse of medications. Providing clinical pharmacy services (e.g., pharmacokinetic dosing, therapeutic interchange, IV/PO conversions, clinical protocols per pharmacy and pharmacist interventions).
