We are looking for a passionate PT or PTA to join our growing Mobile Outpatient Rehabilitation Extension (M.O.R.E) team. This isn’t traditional home health, and it’s not building-bound outpatient. It’s the future of therapy delivery.

What You’ll Do

• Deliver evidence-based, one-on-one therapy in patients’ living environments.

• Build long-term therapeutic relationships to support meaningful, sustainable progress.

• Manage your schedule with support from our interdisciplinary team.

• Collaborate with patients, families, ALF/ILF teams, and community partners.

• Document using a streamlined outpatient EMR designed for mobility.

Qualifications

• Licensed PT or PTA (state license required).

• Prior home-based experience preferred, but not required.

• Strong clinical judgment and comfort working independently.

• Reliable transportation, valid driver’s license, and comfortable traveling in the community.
