Job Details Job Location (Full Address): 601 Elmwood Ave, Rochester, New York, United States of America, 14642 Opening: Worker Subtype: Regular Time Type: Full time Scheduled Weekly Hours: 40 Department: 500130 Psychiatry SMH Addiction Psych Work Shift: UR - Day (United States of America) Range: UR URC 209 Compensation Range: $52,184.00 - $73,053.00 SCHEDULE M-F: 8am-4:30pm Responsibilities Providing Targeted Case Management and Psychiatric Rehabilitation Services across all Strong Recovery clinics. Support wellness and recovery goals of individuals with complex or chronic behavioral health issues through targeted interventions. Case management services to support patients around goals to provide access to services that encourage individuals to resolve problems that interfere with attaining independence, self‑sufficiency, or maintaining themselves in the community. Perform psychosocial rehabilitation services to assist individuals to overcome mental health and substance use barriers. Assist the individual to restore functional level to the fullest possible, focusing on vocational and educational goals. Under general direction perform several care management and psychiatric rehabilitation services consistent with the CCBHC and the Department of Psychiatry policy. Establish and maintain cooperative working relationships with community providers and resources to obtain needed services and support for enrolled patients. Work as part of an interdisciplinary team to support care of patients enrolled in a designated CCBHC outpatient clinic. Essential Functions Conduct assessment of identified patients for psychiatric rehabilitation services. Provide targeted case management services. Serve as a liaison for the treatment team to support activities to engage patients in care, connect them to care management, vocational and education resources, and ensure basic necessities are met. Utilize community and family resources to create sustainable support systems for patients. Review and discuss plan with patient and the treatment team, focusing on linking individuals to clinical and social services within the system and community providers. Design appropriate treatment plan goals according to patients’ psychiatric rehabilitation needs. Conduct evidence-informed treatment with patients. Assess successful completion of goals and plan termination of psychiatric rehab services accordingly. Available to triage case management needs on an urgent basis, including walk‑ins. Personally committed to understanding and meeting the unique needs of external and internal customers. Contribute to the vision and mission of the CCBHC. Maintain thorough knowledge of Department of Psychiatry, CCBHC related policies and procedures. Coordinate with support staff regarding patient scheduling, fees, and insurance issues. Promote hope and recovery by using strengths‑based, culturally appropriate, and person‑centered practices. Collaborate with outpatient team members to provide coordinated care. Assist clinicians as needed to support case management needs. Establish communication using established rules of confidentiality and laws concerning rights of patients, and use correspondence with family, other health‑related agencies, employers, criminal justice programs, and social service status. Maintain active case records including written treatment plans, problem‑oriented progress notes, periodic updates of treatment plans, and appropriate documentation of incidents, discharge summaries, interagency. Complete and maintain professional documentation; documentation of services in the patient’s electronic health record. Maintain professional standards by reading, attending seminars, workshops and conferences. Attend weekly clinical/training/supervision meetings. Participate in departmental and programmatic diversity presentations and meet annual training expectations on diversity. Demonstrate commitment to the mission of the Department of Psychiatry and University by modeling honesty and integrity. Other duties as assigned, including but not limited to: milieu management in clinic area, providing milieu management in reception areas, supporting patients to engage with mental health care, connecting patients to Health Home Care Management Services and other support services, toxicology screen monitoring, assessing patients for physical, behavioral signs of drug/alcohol use and performing BAC tests as needed, assist staff in assessing patients’ behaviors, and develop a plan to help maintain safety of other patients and staff in clinic. Minimum Education & Experience Bachelor’s degree with major coursework in a related health/human service/occupational field required. Master’s degree in an addiction/mental health field preferred. 2‑3 years of relevant experience required. Experience working with adults and/or children and adolescents regarding substance use/mental health disorder concerns, or an equivalent combination of education and experience preferred. Knowledge, Skills and Abilities Experience/Education focused on assisting individuals with vocation/education goals preferred. Licenses and Certifications Must possess and maintain a valid New York State License. Have a satisfactory driving record strongly preferred.
