Position Purpose: The Palliative Care RN, Registered Nurse, is responsible for patients and families in the palliative care program. Provides Palliative care clinical consultation. What You Will Do: Collaborates with manager in the development of performance measurements for the Palliative Care Program to ensure continuous quality improvement; ensures implementation Motivates/supports employees for exemplary performance on internal and external customer service, productivity, and accuracy Provides statistical/budgetary data to appropriate Palliative care program Works collaboratively with all departments to ensure execution of quality delivery of services Ensures that stakeholders are satisfied with services provided; improvement plan developed when necessary Develops tracking outcomes of enrolled patients and ensures information is available for review A commitment to quality palliative care provision and transition to hospice when appropriate Exercises sound clinical judgment in oversight of Palliative Care Program K nows, understands, incorporates, and demonstrates the Trinity Health Mission, Vision, and Values in behaviors, practices, and decisions. Minimum Qualifications: Graduate of an approved nursing education program. Current Registered Nurse licensure in the state of practice. CPR certification 2-3 years acute care nursing and Hospice/Palliative Care certification preferred.
