Carolinas CARE Partnership Mental Health Provider (CoC Program) – Charlotte, NC – Full time The Mental Health Provider is responsible for delivering community-based, trauma-informed therapeutic services to individuals experiencing homelessness under the Continuum of Care (CoC) program. This role provides integrated mental health support in coordination with housing services to promote long-term housing stability and overall wellness. The provider will maintain an active caseload and work collaboratively with a multidisciplinary team to support clients with complex needs. Carolinas CARE Partnership is a nonprofit organization committed to providing safe, stable housing and comprehensive supportive services for individuals impacted by HIV/AIDS, homelessness, and other systemic barriers in the Carolinas. We manage housing programs and services that support our residents in achieving long-term stability, health, and independence. Description Key Responsibilities Direct Clinical Services Maintain an active caseload of approximately 20 clients. Provide community-based, trauma-informed individual therapy to clients experiencing homelessness. Utilize evidence-based practices such as Cognitive Behavioral Therapy (CBT), Motivational Interviewing, and harm-reduction approaches. Integrated Care Coordination Work closely with housing staff to integrate mental health services into housing stability plans. Participate in regular case conferences with Housing Case Managers, Peer Support Specialists, and Addiction Specialists to coordinate care. Client Assessment & Treatment Planning Conduct comprehensive behavioral health assessments to identify client needs and barriers to housing stability. Develop and implement individualized treatment plans with measurable goals. Monitor client progress and update treatment plans as needed. Community-Based Engagement Deliver services in community settings, including client homes, shelters, and other approved locations. Support clients in building skills necessary for long-term housing stability, particularly within transitional housing programs. Documentation & Compliance Maintain accurate, timely, and compliant clinical documentation in the agency’s case management system. Ensure all services align with CoC grant requirements and agency policies. Linkage to Services Connect clients to additional supports, including psychiatric care, substance use treatment, and community resources. Program Support Support program operations and administrative needs as required. Perform other duties as assigned to support the housing and behavioral health programs. Qualifications Education: Master’s degree in Social Work, Counseling, or a related behavioral health field. Licensure: Current or provisional licensure in North Carolina (e.g., LCSW-A, LCSW, LCMHC-A, LCMHC, LCAS, or equivalent). Experience: Minimum of 2 years of experience providing mental health services, preferably with individuals experiencing homelessness, trauma, or co-occurring disorders. Other Requirements: Valid driver’s license and reliable transportation for community-based work.
