Hospice Social Worker

Serenity Hospice provides caring and compassionate hospice care services to patients and families throughout New Jersey. Our mission is to alleviate pain and symptoms; foster a state of calmness and harmony; and encourage an appreciation of family, friends, and life experiences.

Serenity Hospice Care is looking for an enthusiastic and organized S ocial Worker for our Monmouth County areas. This position is full time, Monday through Friday from 8:30am to 4:30pm.

Responsibilities:

• Assist with performing the social services section of the assessment process as well as provide on-going care to patients and their families as needed.

• Assist with performing bereavement services (calls, mailings, documentation).

• Provide updates to the bereavement plan of care as necessary.

• Attend Interdisciplinary Team meetings as needed.

• Managing and completing all psych-social assessments

• Educating the patient and family about services

• Performs other duties as assigned.

Qualifications:

• MSW required, must obtain LSW within 6 months of employement.

• Excellent computer skills, especially Microsoft Office.

• Flexible and able to adapt to changes as needed.

• Able to self-manage and work with teams

• Organized & Detail Orientated.

• Reliable and trustworthy.

• Drivers License with reliable transportation.

This position is full time, Monday through Friday from 8:30am to 4:30pm.
