Hospice Social Worker

Serenity Hospice provides caring and compassionate hospice care services to patients and families throughout New Jersey. Our mission is to alleviate pain and symptoms; foster a state of calmness and harmony; and encourage an appreciation of family, friends, and life experiences.
Serenity Hospice Care is looking for an enthusiastic and organized S ocial Worker for our Monmouth County areas. This position is full time, Monday through Friday from 8:30am to 4:30pm.
Responsibilities:
Assist with performing the social services section of the assessment process as well as provide on-going care to patients and their families as needed.
Assist with performing bereavement services (calls, mailings, documentation).
Provide updates to the bereavement plan of care as necessary.
Attend Interdisciplinary Team meetings as needed.
Managing and completing all psych-social assessments
Educating the patient and family about services
Performs other duties as assigned.
Qualifications:
MSW required, must obtain LSW within 6 months of employement.
Excellent computer skills, especially Microsoft Office.
Flexible and able to adapt to changes as needed.
Able to self-manage and work with teams
Organized & Detail Orientated.
Reliable and trustworthy.
Drivers License with reliable transportation.
This position is full time, Monday through Friday from 8:30am to 4:30pm.
