Physical Therapist - Home Health

McLeod Home Health is seeking a compassionate and dedicated Physical Therapist to provide high-quality care to patients in their homes. This role offers the opportunity to build meaningful relationships with patients and families while helping individuals improve mobility, regain independence, and enhance their quality of life.

Responsibilities

• Provide physical therapy services in accordance with physician orders and established treatment plans.
• Assess patient needs and develop individualized therapy programs.
• Educate patients and caregivers on treatment goals and home exercise programs.
• Collaborate with physicians, nurses, case managers, and other healthcare professionals to ensure coordinated patient care.
• Maintain accurate and timely clinical documentation.
• Participate in performance improvement initiatives, competency training, and professional development activities.
• Serve as a clinical resource and mentor for students, volunteers, and staff as appropriate.
• Ensure compliance with all state, federal, and accreditation standards related to home health services.

Qualifications

• Zero to one year of related experience required.
• Strong communication, organizational, critical thinking, and problem-solving skills.
• Ability to work independently and effectively manage time in a home health environment.
• Knowledge of home health regulations and patient care standards preferred.
• Must possess a current, valid South Carolina driver's license and maintain automobile insurance as required by law.
• Candidates must be at least 21 years of age to be covered under McLeod Health's fleet vehicle insurance policy.

Licensure & Certifications

• Active South Carolina Physical Therapist License required.
• Current American Heart Association Basic Life Support (BLS) Certification required.
