- Manage an assigned caseload of clients
- Coordinate care with clients, families, caregivers, and community partners
- Complete intake assessments and required client visits
- Support caregiver scheduling and coverage needs
- Help address client or staff concerns promptly and professionally
- Healthcare, case management, or caregiving experience preferred
- Strong communication, organization, and problem-solving skills
- Comfortable working independently, in a fast-paced environment, and as part of a team
