Key Responsibilities

• Provide skilled physical therapy evaluation and treatment services to patients in a skilled nursing facility setting
• Develop and implement individualized treatment plans in accordance with physician orders and patient goals
• Document patient progress, treatment plans, and outcomes using facility EMR systems
• Collaborate with interdisciplinary care team to ensure coordinated patient care
• Must be open to floating between assigned worksites as needed

Required Qualifications

• Active California Physical Therapist (PT) license required
• Minimum 1 year of SNF (Skilled Nursing Facility) experience required
• Standard credentialing requirements must be met prior to start

Preferred Experience / Skills

• Experience with Casamba EMR software highly preferred
• Strong documentation and communication skills within a multidisciplinary team environment
