Continuum Hospice in Cape Cod is seeking a Full Time Compassionate Resource Nurse (RN) to provide patient care in homes or facilities, coordinating with intake, referring physicians, and the Hospice Medical Director. The role supports the hospice philosophy of care, conducts pre-assessments, admissions, and education for patients and families, with a Monday–Friday 11 am–7 pm schedule.
