Job Description

We are seeking a dedicated nursing professional responsible for assessing, coordinating, delivering, and evaluating nursing care in an acute patient setting. This role involves providing holistic and individualized care to patients while developing, implementing, and managing a patient care plan that addresses various psychological, sociocultural, spiritual, economic, and lifestyle factors.

Principal Accountabilities

• Assessment and Planning of Patient Care:
  
  
  • Conduct comprehensive assessments of patients and their families, covering physiological, spiritual, and emotional aspects.
  • Collaborate with physicians and other healthcare team members to ensure continuity of patient-centric care plans.
  • Communicate care plans effectively within the team to ensure seamless transitions and updates.
• Provision of Patient Care:
  
  
  • Deliver compassionate, competent, and cost-effective patient care aligned with hospital policies and evidence-based practices.
  • Maintain a safe and therapeutic environment for patients and their families.
  • Delegate tasks appropriately based on team members' qualifications and capabilities.
  • Communicate clearly with patients and their families to ensure understanding of treatments and care processes.
  • Adhere to regulations to ensure the highest level of patient care while protecting patient confidentiality.
• Evaluation of Patient Care:
  
  
  • Monitor the effectiveness of patient care interventions and modify care plans as needed.
  • Provide ongoing education to patients and families throughout their care journey.
  • Document patient responses and care updates accurately in electronic health records.
  • Assist in training and validating competencies for new staff and help orient them as required.

Education and Qualifications

• Graduate from an accredited school of nursing
• Current RN licensure in California
• BLS (Basic Life Support) certification required
• ACLS (Advanced Cardiac Life Support) certification required
• NIHSS (National Institutes of Health Stroke Scale) certification required
• Minimum 2 years of recent relevant nursing experience, with 2 years in MedSurg/Telemetry required
• Experience with EPIC charting system is mandatory

Skills and Knowledge

• Comprehensive understanding of nursing theories, medical terminology, and patient care best practices.
• Effective written and verbal communication skills suitable for diverse audiences.
• Strong organizational and time management abilities, with a focus on prioritizing patient care.
• Proficiency in basic computer applications, including Microsoft Office Suite.
• Ability to work independently and as part of a collaborative healthcare team.
• Critical thinking skills for problem-solving in routine and complex situations.
