Overview

Office Location: Gainesville, FL
Coverage area:  Alachua County, Bradford County
Schedule:   FT, Monday through Friday, 8am-5pm. NO Weekends or Holidays

As a RN Case Manager You will: 
Assess/monitor physical, emotional, and psychological needs of patients
Create hospice care plans that align with the patient's wishes and goals  
Direct nursing care: administering medications, treatments, and interventions  
Provide pain and symptom management
Educate and support the patient’s family and caregivers
Collaborate with interdisciplinary team
Maintain accurate and timely documentation
