PACE RN Care Manager

As a PACE RN Care Manager, you'll develop meaningful relationships with participants over time and become a trusted partner in their care. You'll see participants in multiple settings, including the PACE Center and occasionally in their homes, giving you a broader understanding of their needs, challenges, and goals.

Unlike many nursing roles that focus on a single episode of care, PACE nurses are involved throughout the participant's journey. You'll collaborate closely with physicians, nurse practitioners, social workers, therapists, dietitians, home care staff, specialists, hospitals, nursing facilities, hospice providers, and community partners.

What makes PACE truly different is that the interdisciplinary team doesn't just identify problems. We work together to make decisions and implement solutions. When a participant needs additional support, equipment, services, or interventions, you are part of the team that helps determine the best plan of care and puts that plan into action.

• Weekday hours, 8am to 4:30pm
• No Weekends! No On-Call!

PACE is a unique program on the forefront of trends in healthcare, combining an insurance role (we are the Medicare/Medicaid insurer for our participants) with a comprehensive healthcare provider role (primary care, home health, adult daycare, transportation, all coordinated by an interdisciplinary team).

The primary purpose of your job position is to promote and maintain the health of participants enrolled in Eddy Senior Care through teaching, counseling, and appropriate preventive services. The Registered nurse (RN) is responsible for assessment of health needs and implementation of nursing care plans for these participants. The RN provides direct nursing care to the participants through in-home visits, supervises the day-to-day nursing activities including care provided by home health aides. Such supervision must be in accordance with current state, federal and local standards, guidelines, and regulations that govern our facility, and as may be required by the Nurse Manager of Clinical Operations, to ensure that the highest degree of quality care is maintained at all times.

Requirements:

• NYS RN licensure.
• Two or more years experience.
• Valid NYS Driver's License, clean driving record, and reliable vehicle
• Experience and desire to care for seniors.

Pay Range: $40.00 - $52.81 Pay is based on experience, skills, and education. Exempt positions under the Fair Labor Standards Act (FLSA) will be paid within the base salary equivalent of the stated hourly rates. The pay range may also vary within the stated range based on location.
