Registered Nurse – Home Health Schedule: 8-hour shifts | Guaranteed Hours: 40/week | 09:00 AM to 05:00 PM Setting: Home Health; Location: Indianapolis, IN, 46250 Local Pay rate: Up to $49.00/ hour (W-2) Responsibilities Provide skilled nursing care to patients in their homes. Complete patient assessments, OASIS, and individualized Plans of Care. Coordinate care with physicians, patients/families, and the interdisciplinary team. Provide medication management, wound care, IV/venipuncture, catheter/drain care, and other skilled nursing services. Educate patients/families on medications, treatment plans, safety, and self-care. Maintain accurate clinical documentation and ensure regulatory/payer compliance. Supervise HHA/STNA/LVN staff and participate in care conferences. Provide on-call services as assigned. Required Current RN license. 2 years of Home Health experience. HCHB and OASIS experience. 1–2 years of clinical RN experience. Current CPR certification. Valid driver's license, reliable transportation, and insurance. ADN or Nursing Diploma from an accredited program.
