Care Management: Implement and monitor individualized care plans while documenting and communicating status changes to the medical team. Interdisciplinary Liaison: Serve as the primary coordinator across all clinical disciplines to ensure cohesive and integrated patient care. Order Fulfillment: Accurately process and execute physician and nursing orders to maintain treatment continuity. Discharge Coordination: Evaluate and manage patient discharge needs in collaboration with the healthcare team for safe transitions. Minimum Requirements Licensure / Certification / Registration: Advanced Life Support obtained within 6 months (180 days) of hire date or job transfer date required. (American Heart Association or American Red Cross accepted) BLS Provider obtained within 1 month (30 days) of hire date or job transfer date required. (American Heart Association or American Red Cross accepted) Pediatric Advanced Life Support obtained within 6 months (180 days) of hire date or job transfer date required. (American Heart Association or American Red Cross accepted) Licensed Registered Nurse credentialed from the Tennessee Board of Nursing OR current home state license for multi‑state license recognition 'Compact State' obtained prior to hire date or job transfer date required. Education: Diploma from an accredited school/college of nursing OR required professional licensure at time of hire.
