Job Description
Title: RN Case Manager

Schedule:
Shift: 8:00 AM - 5:00 PM
Days: Monday - Friday
Start time ranges from 8:00 AM - 9:00 AM

Assignment Details:
Contract Length: 13 weeks
Guaranteed Hours: 40 per week

Requirements:
- Active RN license in PA
- BSN required
- 2+ years of acute care Case Manager experience

Roles & Responsibilities:
- Assess and evaluate patient needs to develop care plans.
- Coordinate services and resources to meet patient needs.
- Collaborate with healthcare team members to ensure quality care.
- Monitor patient progress and modify care plans as necessary.
