Registered Nurse (RN) - San Fernando Valley, CA Summary: The Caring Connection is looking for compassionate Registered Nurses (RN) for intermittent home health visits and case management on cases throughout Los Angeles County. The successful Registered Nurse (RN) will love working with home health patients, including with children in certain cases, be comfortable with documentation requirements, and have extensive assistance in the healthcare industry. We are looking for RNs who are organized and responsible, comfortable reassuring patients and their families, and have excellent communication skills. Registered Nurse (RN) Responsibilities: Administer medications and render treatment to home health patients under physician's orders Communicate with physicians, other RNs, LVNs, pharmacists, administrative staff, and other healthcare professionals Collaborate with physicians to create a Plan of Care for each home health patient Create and contribute to the patients' medical records as needed Respond to injuries, concerns, and complaints from home health patients and their families Track and maintain medical supply inventory along with other staff members Counsel patients and family members before, during, and after treatment Facilitate and coordinate care among various disciplines including LVNs, PT/OT/ST/MSW, etc. as needed Complete and submit required documentation according to Agency policies and procedures Other duties as assigned Registered Nurse (RN) Qualifications: Valid CA Registered Nursing license Advanced knowledge of EMRs 1+ year(s) of nursing experience, preferably in home health Willingness to work a flexible schedule and fill-in when needed Pediatric care experience a plus Bilingual Spanish is desirable Excellent bedside manner and communication skills Licensed Vocational Nurse (LVN/LPN) - San Fernando Valley, CA Summary: The Caring Connection is hiring for Licensed Vocational Nurses (LVN/LPN) to join our growing Home Health Agency. Working primarily in the private homes of our patients providing shift care, the LVN/LPN helps fulfill the care requirements of our patients under orders from their physician. Under the direction of the Director of Patient Care Services or RN Case Manager, the successful LVN/LPN will be fully accountable for ensuring that the highest level of competent, compassionate care and safety is delivered to each home health patient. The LVN/LPN should be motivated and ambitious with previous home health experience. The Personal Care Attendant/Caregiver will be responsible for assisting patients with their personal care and hygiene needs, all activities of daily living and instrumental activities of daily living (ADLs/IADLs), accompanying patients to appointments and on errands as needed, health monitoring, and helping to maintain a clean and safe home environment for each patient. The successful Personal Care Attendant/Caregiver will also be required to maintain accurate and current health records on each patient. Personal Care Attendant (PCA/CNA) Responsibilities: Maintain homehealthcare records that include any changes in physician's orders, diet, or personal living activities Assist home care patients in performing range of motion exercises and overseeing medication management based on physician's orders Help with patient meal preparation and feeding, as well as basic housekeeping functions Assist with all activities of daily living and instrumental activities of daily living according to patient's personalized care plan Work with transportation professionals to make sure that patients requiring additional assistance getting in and out of vehicles are able to safely use transportation as needed Deliver weekly reports to office staff and review important changes in the patient's condition Other duties as assigned Personal Care Attendant (PCA/CNA) Qualifications: 1+ year(s) experience as a Personal Care Attendant/Certified Nursing Assistant Bilingual Spanish a plus Prior pediatric experience and/or prior experience with developmentally disabled individuals desirable Ability to work a flexible schedule (Weekends, holidays, and evenings in high demand) Must be able to pass a comprehensive criminal background check, be willing to submit to random drug screening, and be legally authorized to work in the United States We are an EEO/AA employer.
